Getting Contracted
Physicians should submit the following to our Provider Contracting Department via email:
- Letter of Interest
- PDF W-9 or Free-Write W-9
- CV
- List of Locations
- Provider Roster (if applicable)
We will review your information – along with our current network needs – and provide a response to you within 30 days. Your interest is most appreciated, and we look forward to partnering with you.
Please contact our Provider Relations department at 562.860.8771 so we can work with you personally throughout this process and after to provide a personable partnership. Completed documents can be faxed to 562.269.5829.
For physicians who need to update their practice information, such as an address, suite, phone or fax changes, please send the updated information to us via email at CVPG_Provider_Relations@pdtrust.com. Be sure to include the new change, effective date, and updated W-9.
